Our Care Team
The Hospice Team
Clinical Hospice Manager
The Clinical Hospice Manager oversees the day to day running of Hospice and leads with inspiration and loving kindness to manage all Hospice activities, ensuring sustainability, effectiveness and good governance.
Clinical Team
The clinical team is made up of Clinical, Registered and Enrolled Nurses and Patient Care Assistants and are responsible for all clinical care, under the direction of the GP, or Medical Practitioner. Ongoing palliative care training ensures that the team deliver best practice care and stay abreast of any changes that occur in the Palliative Care area of specialty.
Support Services Team
The Support Services Team includes admin, finance, cleaning, catering, and volunteer management to ensure the Hospice operates efficiently and effectively, supporting the clinical care team to provide the best care possible to guests and families.
Research Nurse
The Hospice research nurse conducts clinical research, builds clinical trial capacity, develops research training opportunities and improves community outcomes through the implementation of evidence and innovation in palliative care.
Guest and Family Support
The Hospice has a Guest and Family Support Coordinator who can provide your family/significant others with any support needed whilst in hospice. The Coordinator is assisted by a team of volunteers.
Volunteers
Volunteers are a treasured and dedicated part of the care team. They donate their time, effort and love to provide support and companionship to our patients and families. They assist in all areas of operation including being on the board of management and committees, clerical work, reception, catering support, gardening, fundraising, patient and carer support.
Our Partners in Palliative Care
GP's
Your GP remains the primary provider in your care and they will visit you on a regular basis. If your GP is not available, they will ensure that another GP covers for them. The GP will liaise with other doctors/specialists and may ask them to come and review your care if needed.
Hall and Prior - Clarence Estate
Hospice works closely with Clarence Estate who offer palliative care support services to guests as they transition to Hospice from home/residential care or return to home/residential care after leaving Hospice. Our teams work closely together to ensure our guests have all the support necessary to experience death in the place of their choosing, whether that is in Hospice or at home.
Great Southern Palliative Care Team
Albany Community Hospice partners with the Great Southern Palliative Care Team in order to give you, our patient, the best possible care.
