About Us
Albany Community Hospice has been providing palliative care in the Great Southern Region for over 30 years. As a not-for-profit, eight-bed inpatient service located in Albany, we are dedicated to providing comprehensive palliative care that addresses the physical, emotional, psychological, and spiritual needs of our patients, while also supporting their families and significant others.
About the Role
We are excited to offer an opportunity for a Casual Registered Nurse to join our team. This role involves working autonomously, planning, managing and evaluating patient and family/carer palliative care needs, applying appropriate interventions, and working within an interdisciplinary clinical team structure to provide best practice outcomes for our patients and their loved ones.
Key Responsibilities:
- Provide high quality care based on the assessed clinical needs of the patient, consistent with their values, goals and preferences as documented in their care plan;
- Deliver safe and quality health care in compliance with National Safe and Quality Health Standards, the National Palliative Care Standards, and ACH policies;
- Maintain patient records accurately and in a timely manner;
- Provide effective delegation to Enrolled Nurses and Patient Care Assistants within the scope of their practice;
- Undertake effective and empathetic communication with patients and their carers/families
- Communicate and liaise with internal and external service providers/stakeholders
- Actively participate in quality improvement initiatives and projects
About You
Do you:
- Hold a current Registered Nurse registration with AHPRA
- Have recent experience delivering excellence within acute healthcare, ideally in palliative care
- Demonstrate confidence and competency in making decisions autonomously
- Progressive computer and IT literacy
- Possess advanced interpersonal skills enabling harmonious and positive relationships with colleagues, service providers, patients and families/carers
- Embrace continual professional development in palliative care
