We’re asking the community how we can be better

Albany Community Hospice (ACH) has been delivering expert palliative care to the Great Southern Region for over 30 years. Over that time its changed locations and evolved but at all times its commitment to delivering clinical excellence with loving kindness for people with life limiting conditions has remained at the very core of its operations.

The Board and Management of the Albany Community Hospice are once again looking to the future, and we are seeking your insight.

At ACH we look after ‘guests’ rather than patients, and we also aim to support the families and other close caregivers of our guests.

As somebody who as a guest or a caregiver has experienced ACH’s work we would really appreciate it if you could reflect back on your ACH experience. If you can, please find 10 minutes to complete this survey, the responses to which will be used to inform our strategic planning process.

All individual responses will be de-identified.

The survey will be open from Thursday 10th February and will close midnight on Sunday 27th Feb.
